Community Hearing Care
Project proposal
Working programme framework · 26 October 2026 · Budget and project-specific approvals pending.
Purpose and scope
A screening result alone does not explain the cause of hearing difficulty or establish the right intervention. People need a clear route to assessment, counselling and continued support.
The pathway includes hearing assessment, ENT and audiology review, and clinician-recommended medical, surgical or device-based care. Rehabilitation and follow-up are part of the pathway.
Delivery plan
- Community awareness and recognition of a hearing concern
- ENT and audiology assessment
- An individual care plan explained to the person or family
- Treatment, device support or rehabilitation when indicated
- Follow-up and review of use, communication and remaining needs
Proposed geography: Hisar and nearby communities. Entry may be through outreach, a referral or the support desk. Clinical need is assessed by the treating team; charitable assistance is reviewed separately against need and resources.

- Santosh Cancer Foundation
- Community coordination, review of support needs, approved assistance and programme reporting.

Resources and budget headings

Targets, project duration, budget totals and funding commitments are not yet set in this framework. These must be agreed for each project before implementation.
Monitoring and review
- Unique people screened or referred
- Diagnostic assessments completed
- Recommended interventions and uptake, counted separately
- Documented rehabilitation contacts
- Follow-up status and recorded functional change
Review monthly aggregates with the programme coordinator and clinical team. Record the denominator, reporting period and follow-up completeness before describing an outcome.
