Community Breathing Care
Project proposal
Working programme framework · 26 October 2026 · Budget and project-specific approvals pending.
Purpose and scope
People may not know which service to approach for a persistent nasal or sleep-related concern. A defined pathway connects information, clinical assessment and appropriate follow-up.
The programme covers ENT-related nasal obstruction, allergy or sinus concerns, upper-airway problems and sleep-related breathing concerns. The clinical team determines assessment, treatment and any referral outside the ENT service.
Delivery plan
- Community information and recognition of a concern
- Clinical triage and specialist assessment
- Investigations and an individual care plan when indicated
- Medical or surgical treatment, or onward referral
- Follow-up with documented symptoms and care status
Proposed geography: Hisar and nearby communities. Entry may be through outreach, a referral or the support desk. Clinical need is assessed by the treating team; charitable assistance is reviewed separately against need and resources.
